Director will be submitting a sign in sign out sheet from for all attendees of the training on 3/19/25. This requirement is not me as evidenced by Assistant Director states diaper liners have been purchased and placed in all rooms that provide diaper changing or potty training. Per Assistant Director Staff has been informed that the liners are required for all diaper changes involving children in care moving forward.
Based on interviews, licensee did not comply with the section cited above, at least one child was made to sit bare bottom on the bathroom floor during diaper changes which poses a potential health, safety and personal rights risk to persons in care LPA verified liners in each room. Ms. Gonzales stated they are having a staff meeting Thursday 9/26 and will add personal rights and potty-training procedures to the training. An agenda will be sent along with staff sign in sheet via e-mail to LPA Lane on 9/30/24. Assistant director stated she submitted a work order for emergency pest control service to address the ant situation at the facility (and in clownfish room) and send copy of pest control report via email to LPA Lane by 9/24/24. Based upon LPA observation of ants in Clownfish room and staff interviews, facility has not addressed the ant issue adequately which is a potential health, safety and personal rigths risk to children in care.

Director stated that child C1 is no longer enrolled at the facility. DIrector will provide a written plan to help mitigate challenging behavior in the classroom that supports teachers and prevents multiple injuries from occuring to students and staff. Based upon LPA observation and interviews with reporting party, staff members, parents and children, there was not enough staff supervising to meet the children's needs which is a potential health, safety and personal rights risk to children in care.
