  Director removed the sensory table. Director states as of 8/27/25 the facility started to clean the playground area for the sand to continue to be removed weekly and for upkeep of the outside play area. A hired cleaning crew is currently cleaning the facility once a month. Based on observations, interviews and record review, the facility did not ensure the surface of the playground area was free of hazardous debris which posed a potential Health, Safety or Personal Rights risks to persons in care. The facility stopped placing sand in the holes. Director states will provide the LPA with the new maintenance crew schedule and proof of purchase and work order showing that the holes and hardened areas have been patched and/or replaced by 10/31/25.
