based on observation, interviews, and records review, Staff #1 did not ensure children were at a safe distance prior to closing Licensee stated she will conduct an all staff training with staff on care and supervision. Licensee will send the staff sign in sheet and training material to Community Care Licensing by 12/12/2025. the door on a child's finger causing a severe injury. Staff also did not secure the door closed after the incident, allowing children to exit to the playground without a teacher present which poses an immediate health, safety or personal rights risk to persons in care.
