There were spider webs and dirt covering a play house and a play structure. LPA also observed an unrolled water hose in the outdoor activity space which is a tripping hazard. Site Supervisor will watch Community Care Licensing video titled "Child Care Reporting Requirements." This video can be viewed be accessing the Departments website: ccld.childcarevideos.org. Site Supervisor will email LPA a summary of the video by 12/24/2024. Based on observation, interviews, and records review, the facility did not notify the Department that infants had to be moved to an alternate classroom or report that repairs were needed due to rodents entering a classroom which poses a potential Health and Safety Risk to persons in care.

Site Supervisor had maintenance repair the rodent entry ways. This was completed on 11/25/2024. Site Supervisor will increase pest control visits to the facility and submit documents to LPA showing pest control has come to the faciliity to inspect for rodents by 12/24/2024. Based on observation, interviews, and records review, rodents entered the infant center in several rooms. This posed a potential Health and Safety Risk to persons in care.
