LPA reviewed and provided verbal and written guidance on ratio and capacity requirements for her license. September 2022, she cared for infants #1,#2,#3 and #4 as well as child #5 alone for approximately 20 minutes causing her to be out of ratio. This poses a potential health, safety and or personal rights risks to persons in care. Today, Licensee completed a written statement indicating she understood the requirement to follow all ratio and capacity requirements when operating her facility with and without an assistant. Deficiency cleared today.

Deficient Practice Statement Based on observation the licensee did not comply with the section cited above. Licensee sits next to the infants while they sleep in cots on the ground. A record review indicates that infant #1 and infant #2 were recently enrolled and licensee stated she was aware she needed a crib and she is in the process of obtaining a crib.
