evidenced by Per licensee, she has updated her parent contract to indictae that the provider is not providing transportation to and from school. LPA's observed that Licensee's contract for parents state that transportation from school is the parents responsibility. LPA's obtained a copy of new contract during visit. Based on interviews, licensee did not pick up school aged children from school when parent/provider agreement service was agreed upon, which poses/posed a health and safety risk to children in care. LPA's advised that amendment to parent contract needs to be provided to all families in care and have parents sign receipt.

Per Licensee will install a clear partitioner in the open wall space between the kitchen and daycare room to prevent animals from entering the day care space. Licensee will provide a picture of secured installment by POC due date. Per Licensee, will submit a written schedule to maintain regular cleaning of the facility to include deep cleaning of the bedroom and kitchen where the animals reside during day care hours. Per Licensee, facility will be cleaned 3 times a day, morning, afternoon and evening. Based on LPA observation, the licensee did not comply with the section cited above, facility has an unpleasant odor due to the numerous pets in the facility. Facility was observed to have four animals(cats/dogs) in the kitchen, eight animals in a bedroom(cats/dogs) and an additional eight animals in the garage(peacocks). LPA noticed a strong unpleasant odor due to the numerous pets in the facility. which poses/posed a potential health, safety or personal rights risk to persons in care. LPA’s observed a total of seven pets in the facility on this day. During previous visit on 10/24/2023, LPA’s observed a total of twenty pets.
