Facility director is to arrange the outside children furniture and play ground equipment in such a way that it does not propose a hazard from conflicting activities. Facility director is to obtain safety padding to go around the wooden pillars and concrete base in the patio play area. Based on interviews, observations and records review, a child was injured in the outdoor play area from falling and hitting the pillars which poses/posed a potential health, safety or personal rights risk to persons in care. Director is to submit to LPA within (5) days for review of contractors quote, and completed work of padding installation to be completed within 30 days, if more time required director will request an extension in writing to the LPA. 101238(a) · 101238 Buildings and Grounds (a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors.... LPA observed the facility has shut down the water fountain and is completely covered.There is a "Do Not Use" sign on the fountain, until it is repaired/removed. The children are currently using personal water bottles that are refilled throughout the day the facility has filitered water on site. Based on lead testing results,1 indoor water fountain that was tested have lead levels exceeding 5.5ppb which poses a potential Health and Safety risk to children in care. DIrector must show proof to the LPA via email/photos, that the water fountain has been removed and/or repaired. The documentation must be submitted to LPA no later then March 8, 2023.
