POC: Director will seek an outside vendor for in depth training on "Care and Supervision" for all staff & provide scheduled training information to LPA by Friday, 5/23/25. After the training, director & staff is to provide detailed written documents of their understanding, plan of prevention along with signatures & dates of all attendees. Based on LPAs interviews and record review, staff did not comply with the section cited above as a child was left behind on 2/21/25 in a classroom & 3/6/25 unsupervised in the outdoor playarea, which posed an immediate threat to the health and safety of the children in care. An immediate civil penalty of $250 is assessed today due to a repeated violation with in a 12 month period. 101238(a) · 101239(a) Building and grounds- the child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well being of children, employees and visitors. The director states that the plan of correction is that the facility does not use the 2 water fountains, children bring their water bottles from home and another area is used to refill the water bottles as needed. The sink "A" is only used for hand washing. Report has been posted and parents notifed per director.
Based on observation and documentation of the Lead Testing Result, the facility has 3 (three) water outlets that exceeds the 5PPB level Director submitted via email photos and report summary. Director will update LPA via email by April 20, 2023 if the facility will remove and/or replace the two outdoor water fountains. The licensee shall provide care and supervision as necessary to meet the children's needs Effective 09/07/2022 Director will review contracts of children and make sure the facility is able to accomdate. Director is in the process of hiring additional staff. Director's goal is to overstaff to ensure there is enough staff. Based on the LPA obersavations and staff interviews, and documents reviewed. This requirement was not met as evidence. This poses a potential risk to the Health, Safety and personal rights of the children in care. POC is cleared during today's visit.
