A REVIEW OF RECORDS AND INTERVIEWS WHICH REVEALED A CHILD WAS ALLOWED TO WALK OUTSIDE TO THE PLAYGROUND WITH ANOTHER CHILD'S FAMILY The facility has terminated employment of the employee that allowed the child to walk out with another child's family, and implemented a new policy to station only employees that are familiar with the children in care to be at the door during transition. Outlet has been permanently removed. Director has submitted all documentation for lead testing. This deficiency was cleared during the inspection. Letter of Clearance was provided. Per LPA's inspection there is at least one outlet in the kitchen area that exceeded action level established by State for lead exposure. Children used the outlet for drinking water.
