Per investigation, on 3/7/23 C1 sustained head injury while playing in outdoor yard.S1,S2 were present but did not visually see incident happen.Lack of supervision resulted in child's head injury that required medical attention and failure to prevent imminent dangerous situation, not provide active 100% visual supervision posed an immediate risk to health and safety of children in care. Repeat violation within 12 months, civil penalty of $250 assessed today.

By POC due date 6/8/23 Director agreed to submit a written plan on how facility will be in compliance with this regulation moving forward Based on investigation, on 3/28/23 C1 had frequent bowel movements while in care, used bathroom several times. S1, S2 stated they helped C1 few times, but were unaware clothes were soiled. Soiled clothes were noticed by parent at home and facility was notified. Staff stated they were unsure how many times C1 used bathroom or how many times staff provided help. Toilet chart shows the child used bathroom 2 times, although S1,S2 stated the child went more than 2 times. Photos of soiled clothes were provided as proof

By Due Date 4/17/23 hold all staff meeting, discuss plan, regulations, watch training video on www.ccld.ca.gov. Send agenda/attendance verification to CCLD by end of 4/17/23. Per investigation, S1 left C1, C2 in Lake room alone. There was no one in the room and S1 did not hand them over to a staff person. This posed an immediate risk to safety of children. Provide a copy of this licensing report to parents/guardians of all children currently enrolled and to any newly enrolled parents/guardians for 12 months. Exterminator service came today on 3/22/23. No droppings have been observed today. This citation was cleared during visit. Director understands if such an incident happens again, affected areas should be made Per investigation, rodent and droppings were observed in Toddler Room on 3/17/23, 3/20, 3/21. Facility failed to close the room, staff cleaned droppings and equipment in the morning and children continued to have access to the room during this period. Exterminator service did not come in until today. This posed a potential risk to health/safety of children in care. inaccessible to children until professional services are called and space is deemed safe and sanitary for use.

Per LPAs investigation Child 1's enrollment was terminated without any prior notice, therefore not abiding by Parent Handbook and Admission Agreement contract which states 7 day notice shall be given.
