Director agreed to submit training agenda, staff declarations stating what they have learned, and staff sign in sheet by 5pm on 12/19/24. Based on interviews, on 1/10/24 at approximately 1:30pm, Staff 4 (S4) left a group of children in school age room 2 unattended for approximately 2 minutes. This is an immediate risk to the health, safety, and personal rights to children in care.
