Facility contracted Dewey Pest control on 9/25/2025. Dewey has placed mouse traps at the facility. LPA received pest control documentation. Based on observations, record review, and interview, LPA observed rodent droppings in the green room near the cots and blue room near the infant's cribs and soft play area which poses an immediate health and safety risk to children in care.
