Per Esther Quintero Blake, Educational Area Supervisor the LIC9182 will be submitted to the Department via fax for Staff #1-5 by POC due date 06/21/23. During LPA's inspection LPA called the Regional Office and determined through DSS Guardian Background Check System and Licensing Information System (LIS) that Staff #1-5 are fingerprint cleared; however, they are not associated to the facility. This poses a potential health and safety risk to children in care.

101212(d) · Reporting Requirements: (d) Upon the occurrence, during the operation of the child care center of any of the events specified in (d)(1) below, a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours.
