101212(b) · Reporting Requirements. (b) The name of the child care center director, and any fully qualified teacher(s) designated to act in the child care center director's absence, shall be reported to the Department within 10 days of a change of child care center director or designee(s).
Acting center director, Anais Heim stated that the facility will submit a written plan to the Department reporting the change and information on how it intends to meet Title 22 regulations during this transition period. Additionally, the facility will submit an updated LIC500 and LIC610 to the Department by 4/25/25. Based on an interview with the acting center director, Anais Heim on 4/11/25, it was corroborated that the facility's former center director, Melissa Terhune is no longer employed by this facility effective 03/14/25. The Department has not been notified of this change. This poses a potential risk to persons in care. Deficient Practice Statement Based on the LPA's observations on 08/05/24 at 02:15pm, the LPA observed that at least 15 napping preschool children did not have a sheet to cover their cot during nap time. The assistant director, Anais Heim informed the LPA that sheets are available for every napping child at the center, however, staff are not using the sheets on all cots during nap time. 101223(a)(3) · Personal Rights (a)The licensee shall ensure that each child is accorded the following personal rights: (3) To be free from corporal or unusual punishment, infliction of pain, humiliation, intimidation, ridicule, coercion, threat, mental abuse, or other actions of a punitive nature including but not limited to: interference with functions of daily living including eating, sleeping or toileting; or withholding of shelter, clothing, medication or aids to physical functioning.
Interim Director Brenda Bustmante stated that staff S2 is no longer employeed by the center. Bustamante stated that during the center's next all staff meeting on 11/30/23, staff will watch the Children's Personal Rights in Care video on the CCLD website https://ccld.childcarevideos.org/child-care-center-operators/childrens-personal-rights-in-child-care/ and review the written regulation during their all staff meeting. A sign in sheet with staff names and signatures to verify training will be submitted to the LPA via email by COB 11/30/23. Based on interviews conducted and documents obtained, it was corroborated that staff S2 roughly handled child C1 as C1 attempted to sit up in their cot during nap time. 101700.3(b)(1) · 101700.3(b)(1) A result with values of 5.5 ppb or greater shall be deemed an Action Level Exceedance. LPA observed that the facility has temporarily removed the affected faucet “I” from service. The facility has replaced the faucet and will retest and submit test results to LPA at yang.yang@dss.ca.gov by 03/02/23. Based on a record review, the facility had one faucet (Site “I”) that exceeded the allowable levels of lead in the water. Site I had a reading of 6.7ppb. This is a potential health and safety risk to children in care. Center Director Meronek stated children in care are receiving drinking water from other unaffected faucets at the facility.
