The LPA printed out a copy of regulation CCR 102423(a)(2) for the licensee and staff S2 to review and sign to confirm that they are familiar with these regulations. Additionally, the licensee informed the LPA that child C1 is no longer enrolled in her program.

  Based on the licensee’s own admission and statements provided by the licensee’s staff S2, It was confirmed that child C1 was not changed in a timely manner on at least two separate occasions while in care. This poses/posed a potential health, safety and/or personal rights risk to the child(ren) in care
