Each personnel record shall contain the following information: (11) A health screening…(12) Tuberculosis test documents… This requirement has not been met as evidenced by Licensee will have S3, S4, & S5 provide a health screening and will send proof of health screening to LPA via email. Licensee will have S3 & S4 provide proof of a negative tuberculosis test and wills end proof to LPA via email.
