  evidence by Facility has already fixed the latch and chains on the gate so that no child would be able to fit through the space. The facility will also provide a training to staff on children's personal rights, and the importance of supervision and providing a safe environment indoor and outdoor for children in care. Based on the interviews and observation of the yard, there is sufficient information indicating that Child #1’s personal rights were being violated as the facility did not provide a safe environment while playing in the outdoor preschool play yard. This incident posed an immediate risk to persons in care. Additionally, facility will submit a plan to CCLD outlinging a plan in writing on how they will prevent something like this from happening in the future.
