Per Lead Teacher, they will have the water outlet replaced and retested. Lead teacher will have PUSD submit proof of work order for replacement and confirmation of new water sampling appointment, by POC date. Based on interview and record review, licensee did not ensure that Action Level Exceedance in Water Outlet C, was made inaccessible to children in care.
