evidenced by Per director, she will have a staff meeting to discuss the severity of it and have staff sign in. Director will place reminder posters with guidance near changing table for visual reminder. Per director, she terminated one employee that she was aware who was doing this. Based on interviews, staff was observed to leave child unattended while on changing table, which poses an immediate risk to the health and safety of children in care. Per director, she will submit staff meeting agenda and staff sign to LPA S Li by 12/10/25.

Based on interview and record review, Per Director, they will conduct a staff meeting regarding facility procedures and have all staff sign an attendance form. Written declarations for all staff will be submitted noting that all staff will adhere to facility and licensing policies and regulations, while on campus. Licensee did not ensure that prohibition of tobacco product, in the form of an e-cigarette, was upheld on property. Staff was found to have used an e-cigarette in the facility office, after hours.
