Actions already taken by facility and director: notified parent of child involved, spoke with staff involved to discuss and reflect on the incident, completed retraining of practices and procedures of transitions with staff. Director will address this incident with all staff at a later time. Based on observation and interview, the licensee did not comply with the section cited above as one (1) child was left unsupervised, which poses an immediate/potential health, safety, and/or personal rights risk to persons in care. LPA will follow-up with Director Christina De Vera via telephone call or email as she is not on-site during today's inspection.
