  The facility has already taken the following actions: an in-house investigation was completed and documented, staff who handled child roughly has been terminated, all-staff meeting was held to address how to safely provide care and redirect children. Based on observation and interviews, the licensee did not comply with the section cited above as statements from interviews and camera footage supported the allegation of staff handling a child roughly, which poses an potential health, safety, and/or personal rights risk to persons in care. LPA determined the actions taken by facility are sufficient to clear the citation. LPA recommended for director to review what the Personal Rights of children and Mandated Reporter requirements are with all staff members.
