The Director submitted a UIR for this incident on 02/12/26, following the initial 10-day complaint investigation completed on 02/06/26. The Director will also provide a written statement outlining the steps that will be taken to prevent this incident from reoccuring. It was revealed that the facility did not follow up with Parent #1 to confirm receipt of a message sent through the Pro Care app at approximately 10:00 AM, when Child #1 was initially injured. Parent #1 became aware of the message only after calling the facility regarding a separate issue. This delay in communication may pose a potential health and safety risk to children in care.
