101227(a)(7)(B) · 101227 Food Services (a)(7)(B) A child shall not be served any food to which the child's record indicates he/she has an allergy.
Per Director, an allergy plan, a transition plan and child specific plans were created on 5/28/2025. All staff were given one on one trainings and changes that were included in the plans were implemented in the same week of the incident occuring. Based on interview and record review, a staff member on site provided an enrolled child a food the child was allergic to, during hours of operation. This poses a potential health, safety or personal rights risk to children in care. The changes included a separate table for C1 and a red table mat with a red plate for C1’s allergy identification. Director maintains regular communication with parents of C1. All staff will receive an updated CPR and First Aid training in August 2025. Deficiency cleared as of today 6/18/2025. 101238(a)(1) · 101238 Buildings and Grounds (a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. (1) The licensee shall take measures to keep the center free of flies, other insects, and rodents. The Director should provide the measures to the Licensing Office to keep the facility free of rodents on or before February 02, 2024.
Based on observation, interview and records review, the licensee did not comply with the section cited above that the videos and pictures demonstrating the dead rodent in the facility, which pose a potential health, safety, or personal rights risk to persons in care. The Director has appointed a contractor to keep the facility free of rodents. The deficiency was cleared during the visit on January 18, 2024.
