Based on records review of video footages and photos, an infant was pushing another’s infant head and infants were eating food off the floor without staff acknowledging it. Based on review of video footage dated 2/06/26, LPA observed (S6) to use their hand to grab and push C1 away multiple times. This poses an immediate risk to the health, safety, and personal rights of children in care.

LPA Navar received a copy of a Diaper Schedule that has been implemented since 1/16/2025. Based on records review: LPA reviewed records of C1 being in care for at least 7.5 hours on 1/14/2025 with 1 documented diaper change at 11: 00AM.This poses as an immediate risk to the health to the children in care.

Based on review of video footage dated 9/09/24, LPA observed (S3) at 1:20PM use both their hands to lift up C2 by the armpit area and then laid C2’s body down by force with feet hitting cot first, knees, then upper body. This posees as an immediate risk to the health, safety, and personal rights of children in care.
