Based on LPA’s interviews, S1 stated observing S4 yell at children. S8 disclosed that it was reported to S8 that S4 was speaking too loudly to children in care so S8 spoke to S4 about S4’s tone. C4 disclosed C4’s teacher yells at them (children) when they get in trouble Deficiency Dismissed

101239(m)(1) · Fixtures, Furniture, Equipment and Supplies-(m) All play equipment and materials used by children shall be age-appropriate.(1) The licensee shall provide a variety of age-appropriate equipment, toys and materials in good condition and After touring the facility with LPA, Director and Head of Schools agreed that the play kitchen should be immediately disposed. Facility disposed the wooden play kitchen on 5/23/23. Administation stated since that day they did an assessment of their play yard and beautified the space. in sufficient quantity to allow children present to fully participate in planned activities.

Based on LPA’s interviews, observations and photographs taken while touring the facility, shows a wooden play kitchen with mold and in disrepair They also assigned a staff member to do daily checks of the outdoor space to ensure everything is in good condition and ready for the children to use.

Based on observation and interview, file review, staff Ophelie Minnigerode and Tiphaine Untalan did not have background clearance. This is an immediate safety risk to the the children in care. Staff with "PENDING" status cannot return to facility until overall status has been cleared eligible to work. Director will email signed LIC 9224 forms to LPA at dianna.valdezsantana@dss.ca.gov
