In addition, director held a staff meeting on June 7, addressing topics like incident reports, communication with children and parents. Director, will email LPA meeting agenda and sign in.

Based on LPA’s interviews, 4 out of 6 staff stated S5 treated children in rough manner. This poses an immediate risk to the health of the children

101229(a)(1) · Responsibility for Providing Care and Supervision: No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. Director stated Director will also share a "care and supervision" video with staff for them to watch and discuss by POC due date.

Based on LPA’s interviews, 1 out of 6 staff disclosed C3 was bit on the leg by C4. C3’s parent came to S7. S7 admitted staff did not see the incident occur or realize at the time that C4 bit C3 on the leg. This poses an immediate risk to the health of the children
