LPAs observation. During today's inspection, LPAs observed serveral playground equipment not anchored in. LPA was able to lift and move the equipment By 07/16/2025 Facility will either remove or anchor the playground equipment. Facility will submit a statement of their plan to prevent children from utilizing play equipment until Community Care Licensing inspects play equipment. LPAs also observed a child riding a spinning tricycle that was not properly anchored in. This poses a potential health and safety risk to children in care.
