There was a BB gun and alcohol stored in the bottom shelf of a closet in the hallway leading to the backyard. This poses an immediate health, safety or personal rights risk to persons in care. The licensee shared with LPA that she is part of a group where providers share information with one another. A piece of information shared was that 15-minute checks were no longer required.
