101223(a)(3) · Personal Rights: To be free from corporal or unusual punishment, infliction of pain, humiliation, intimidation, ridicule, coercion, threat, mental abuse or other actions of a punitive nature including but not limited to: interference with functions of daily living including eating, sleeping or toileting; Assistant Director agrees to conduct a training with staff regarding how to handle and/or manage similar situations. Agenda and sign in sheet to be submitted to the Riverside Child Care Regional Office by 1/5/2023. or withholding of shelter, clothing, medication or aids to physical functioning. This requirement was not being met as evidenced by the information obtained that a staff slapped or hit a child in the mouth area. This poses an immediate health, safety, or personal rights risk to children in care.

101238(a) · Buildings and Grounds 101238(a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors.

Facility will complete deep clean of all classrooms and submit proof with pictures to the Department by POC due date 01/06/22. Based on LPAs observations the facility is not meeting the section above in that dirt/grime was observed on floors, toilets, tables and trash can. Facility will implement corrective action pursuant to section CCR 101704 for immediate cessation of outlets testing with action level exceedance until it is replaced and retested pursuant to section Based on records review of required lead testing, the facility had lead values of 5.5 or above on water outlets: E (7.1 pbb). This is a potential health and safety risk to persons in care. CCR 101705 and returns a result at or below the Acton level. Facility will notify CDSS with completion within 30 days.
