, Director stated the teachers in Twaddler A have been terminated. Director also stated that they had conducted a Supervision, Transitioning and Name to Face training with all staff on 10/13/25. Director provided copies from training to LPA during visit. Based on interviews conducted 6 of 6 staff members confirmed that Child #1 (C1) was left on the preschool playground without the supervision from their assigned Twaddler A teachers for up to 6 minutes. This is a potential health and safety risk to the children in care. Director will provide meeting agenda and sign in sheet to LPA. Based on confidential interviews, the facility has on multiple occasions served C1, whom has a food allergy, food items containing the specified allergen, which poses a potential health, safety or personal rights risk to children in care.

Director will submit a copy of a memo to parents reminding them of sign in/sign out requirement. Director will also submit a copy of a staff training regarding sign in/sign out procedure and requirements. LPAs and LPM observed that number of children present did not match the number of children signed in. This poses a potential risk to the health and safety of chidlren in care. Assistant Director agrees to submit a written schedule of all staff that will be available in the Preschool program in morning hours of 7:00 am to 8:00 am to ensure complaince with staff ratios. Submit a copy to the Department by 11/19/2021. Based on information obtained Staff have been left alone, without an aide with as many as 18 children in the hours of 7:00 am to 8:00 am. This is a potential health and safety risk to children in care if not corrected.
