, Director stated they have already conducted a safety training with all the drivers on 2/9/26 and will provide copies of training to LPA via email. Based on photos sumitted to CCL it was observed that a child was turned backwards hanging over set looking out the back door that was open while van was in motion. This is an immediate health and safety risk to children in care.

Deficient Practice Statement Based on record review, the licensee did not comply with the section cited above in 2 out of 3 staff members do not meet teacher qualifications which poses/posed a potential health, safety or personal rights risk to persons in care. POC Due Date: 02/21/2025

as edvidenced by Director agrees to have all bathrooms deep-cleaned and sanitized. Once completed, Director will contact LPA and LPA will conduct POC visit. Based on LPAs observation, one of the children's restrooms had a strong odor of urine upon entrance. LPAs could smell the urine through mask use. This poses a potenial risk to the health and safety children in care if not corrected.

Director agrees to have the carpet in Rooms 1 and 2 replaced and the carpet in Room 3 deep cleaned. Once completed, Director will contact LPA and LPA will conduct POC visit. Based on LPAs observation, in Rooms 1 and 2, there were multiple rips and tears in the carpets, which can be a tripping hazard. Additionally, the carpet in Room 3 showed signs of wear and tear and had a big, dark and dirty stain on it. This poses a potenial risk to the health and safety children in care if not corrected.
