as Director stated that they had conducted a Transitioning and Name to Face training with all staff on 9/29/25 and 10/13/25. Director provided copies from training to LPA during visit. Based on interviews conducted 6 of 6 staff members confirmed that P1 stated they brought C1 into the classroom from the hallway where they were alone. Witnesses interviewed stated they found C1 in the hallway alone and took C1 back into their classroom. This is a potential health and safety risk to the children in care.
