    , Owner/Director stated they have added alarms to each door and will conduct a supervision training and plan with staff and will submit proof of completion via email to LPA by 12/29/25. Director stated that S2 no longer works at facility. Based on interviews conducted and video footage, on 4/17/25 C1 left classroom into parking lot 2 times and S1 left 3 other children in the classroom unsupervised 2 times. This poses an immediate health and safety risk to the children in care.
