Licensee will also provide a written declaration to LPA, stating understanding of said regulation including a printout of training provided including a sign-in sheet. Based on interviews and record review the Licensee did not provide information to P1 of C1 requested personal health information, records of injuries and center fee information from the CCC and information was not shared with P1 because of a temporary restraining order on file and not a court order. Confidential interviews revealed that information was only given to P2 and not P1, without permission of P2 which is a potential health and safety risk to the children in care.
