  The facility will follow up with their program operation manager to reduce the lead levels in the affected faucets, and will submit documentation showing that lead is no longer present in concentrations over the action level of 5 parts per billion by POC date 10/30/25. Facility may submit documents via email at Mario.Caro@dss.ca.gov. Based on record review, the Facility did not comply with the section cited above because Facility failed to maintain a lead value at or below the Action Level for water lead testing which posed a potential health, safety or personal rights risk to persons in care.
