Based on interviews and record reviews the facility did not comply with the section cited above when LPA conducted a visit on 11/13/2025 and By 12/12/2025 Close of Business, the facility will submit a written statement to LPA, explaining how the facility will remain in compliance. LPA was also informed that facility is currently in the process of interviewing for a Director. LPA was told that S1 was an "Acting Director" until the position was filled, which S1 later informed LPA that they were just a Teacher and the facility currently did not have a Director. During this period multiple incidents have occured which poses an immediate risk to the health, safety or personal rights of children in care. Failure to correct will result in a $100 per day civil penalty until corrected. Repeat violations are $250 per violation and $100 per day until corrected.

By 10/06/2023 submit a written statement on how the facility will never leave a child without supervision of a teacher. ex: Have more staff in the morning. LPA observed the children in the classroom alone, while staff was in another classroom on two separate occasions, which is a potential risk to the health, safety, and personal risk to the children in care.
