Per Licensee, a plan of supervision will be provided to the Department by 2/8/24. Plan of supervision will include preparing snack after nap time, having one adult available to supervise toddler napping areas and supervising children when they use the restroom. Based on interviews, Child 2 was not being properly supervised when the incident occurred as there was a moment where both the Licensee and Assistant did not know where Child 2 was during nap time which poses an immediate risk to the safety of children in care.

Per Licensee, Assistant provider will stay in the infant nap room when infants are present. Assistant provider will sign in and out of nap room and a log will be kept. Licensee will provide proof of sign in/out sheet for 2/7/24 and 2/8/24 to LPA. Based on interviews,Licensee did not hear Child 1 cry when the incident on 10/11/23 occurred until she entered the room. Licensee was in the kitchen at the front of the home and Child 1 was in the in the back of the home which poses an immediate risk to the personal rights to children in care. Licensee was advised that DOJ notice in staff file states that Adult #1's fingerprints were rejected on 8/8/23 and they must be re-fingerprinted. LPAs observed Adult #1 leave the facility at 12:23PM. Per Licensee, Adult #1 will obtain a fingerprint clearance on 10/17/23. Licensee did not ensure that Adult #1 had a completed criminal record clearance. Adult #1 was observed providing care and supervision to children per Licensee adult 1has been at the facility for one month. This poses an immediate risk to the safety of children in care.
