101238(a)(1) · Amended report dated 4/8/26 to correct the deficiency. Buildings and Grounds (1) The licensee shall take measures to keep the center free of flies, other insects, and rodents. This requirement was not as evidenced by The Director will supply proof of the company coming out monthly to control the pest. Director will have the pest control come this month April and in May and send proof by POC date.

101223(a)(3) · Personal Rights. The licensee shall ensure that each child is accorded the following personal rights: To be free from corporal or unusual punishment, infliction of pain, humiliation, intimidation, ridicule, coercion, threat, mental abuse or other actions of a punitive nature including but not limited to: interference with functions of daily living including eating, sleeping or toileting; or withholding of shelter, clothing, medication or aids to physical functioning.

The Assistant Director disclosed that Staff 2 made a disclosure of Staff 1 mishandling a Child in a rough manner. It was also disclosed that Staff 1 was removed from their position temporarily and placed in the office and assigned office related work. Counseling was provided to Staff 1 regarding handling children in a rough manner. During the interview with Staff 3, it was disclosed that Staff 3 has observed Staff 1 on multiple occasions handling children in a rough manner. This poses a potential health, safety, and personal rights to children in care.
