Deficient Practice Statement based on LPA observation, Licensee did not meet above requirement. LPA observed a variety of toys in outdoor play area not maintained such as plastic play equipment to be cracked and have broken pieces exposing sharp edges, sensory table arch way leaning with bent metal bar, and bicycles and scooters to be missing handle bars and rusted at time of inspection.
