Deficient Practice Statement Based on observation, the licensee did not comply with the section cited above in LPA and LPM observed 6 cement stone sculptures that are on the outdoor play area. LPM was able to move one with their pinky and index finger that was on the edge of a garden bed that can fall on a child. There is a metal rod that sticks out of the ground that a child can fall on. There is a small electrical box that has metal protruding on the sides that have pointy sharp corners. There is another area with metal pipes protruding from the ground and 2 unraveled water hoses that are a tripping hazard, which poses an immediate health, safety or personal rights risk to persons in care.
