Director stated they will submit the written LIC624 unsual incident report of the incident to licensing office by 7/12/23. Director will make sure to report any unusual incidents to licensing within 24 hours of occurance and a written report within 7 days to licensing office. LPA provided a copy of regulations. Based on interviews conducted with S1. S1 was not aware and did not report the incident to the licensing department within 24 hours and submit the incident report withing 7 days. This poses a potential health and safety to the childcare children.

While touring the facility LPA Observed staff #1 caring for children inside the Preschool Classroom. Staff #1 stated she has been working at Tutor Time for approximately 2-3 months. Director states she will continued to send out communication to the families and post something different in their lobby regarding the requirement masks for children ages 2 and older. During today's visit it was observed that the Director had extra masks for children at the front and children have their mask on. Director will email the communication to LPA Nguyen by 11/05/21. Based on LPA observations, document reviews and interviews which were conducted. Most facility children did not wear face coverings while in the facility as required by the CA Dept. of Public Health Guidance. This is a potential risk to the health and safety of children in care.
