  a 4-year-old child (C1) found a Nicotine Vapor Pen on the playground and put it in their mouth The director agreed to review the importance of observing the behavior of children continually with staff. And reviewing/updating the procedure of keeping personal items that might be a hazard to children inaccessible to children with staff. S1 unintentionally, forgot to secure their fanny pack in the locked closet, and did not realize vapor pen had fallen in the sandbox. Staff on the playground did not observe the behavior of the child adequately to prevent C1 putting it in their mouth. This is a potential risk to the health and safety of children in care. The director agreed to send a statement with staff’s signatures acknowledging the above licensing requirement to LPA by the due date of 6/6/2025.
