No child(ren) shall be left without the supervision of a teacher at any time.... Supervision shall include visual observation. Director stated that teachers will be going into the building with the children while they use the bathroom. Director stated this has change has already taken place. Director will also sent LPA a written statement with the changes that have been made to melinda.mohr@dss.ca.gov

  Based on interviews, and observations, evidence shows that children are going into the bathroom inside the building alone and without staff supervision. This poses a potential health, safety and/or personal rights risk to the children in care
