S1 stated they are having a site meeting today and will have a training on how to deal with frustrations and challenging behaviors with children. LPA also gave S1 the 'Tell Me What to Do Instead' handout to discuss with staff. S1 will email LPA a signed document from all staff they understand the discussion and have recieved the handout.

S1 and S2 provided proof of an updated "Child Head Count" procedure to LPA and it was provided to all staff at their site meeting on 12/11/2023 and 01/10/2024. Facility has made “Site A, Site H & Site R” inaccessible and notified all parents or guardians of the lead testing results. The facility will submit an LIC 9275, LIC 9276, and Facility Sketch. Director stated Site “A” and Site “H” have already been repleaced & retested and Site “R” will be removed from the premises. Based on record review, the facility had three faucets (Site “A, H, R”), that exceeded that allowable levels of lead in the water (8.9 ppb, 29.00 ppb and 33.00 ppb). This is a potential health and safety risk to children in care.
