This was not met as evidenced by SS stated that she will review supervison handouts provided to her by LPA Jen Patel and have all staff sign off that they have reviewed and provided a copy to Jennifer.patel@dss.ca.gov. Based on LPA observation and interviews, on 3/11/2026 staff did not have visual supervision of C1 and C2 resulting in C2 pushing over a bookshelf on to C1 which poses a potential risk to the health, safety, and personal rights of persons in care.

Which was not met as evidenced by SS reported that since the incident, all furniture, including bookshelves, have been anchored to the wall using L shaped brackets. SS will complete a daily inspection of the furniture until 7/1/26 and provide LPA Jen Patel with a report of the daily inspections by POC due date, 7/2/26. Based on LPA interview, facility did not ensure bookshelfs (31”high x 12”wide x 48”long) were properly secured resulting in one bookshelf falling onto C1 which poses a potential risk to the health, safety, and personal rights of persons in care.

S1 stated they had a staff meeting and went over supervision. S1 stated she has mad a log for batteries and the door alarms making sure they are changed regularly. S1 also stated she has contacted the maintanence manager regarding putting a gate at the cubbies by the main classroom door that will prevent children leaving the classroom. Based on interviews C1 was left without staff supervision for 5 minutes while leaving their classroom alone, walking through the courtyard and into another classroom on 02/13/2024 which posed an immediate health, safety and/or personal rights risk to the child in care. LPA gave S1 the 'Active Supervision' and 'Active Supervision Toolkit' handouts. S1 stated she will go over the handouts with staff at the next staff meeting.

Site Supervisor has had multiple staff meetings regarding supervision, and has added signage to the front gate about latching the gate when going in and out. Site Supervisor has also sent out email to families, and had parents meetings reminding families to latch the main gate. Based on interviews it was revealed that C1 did leave the classroom unnoticed and wandered into the parking lot.

The licensee shall ensure that each child is accorded the following personal rights: To be free from corporal or unusual punishment, infliction of pain, humiliation, intimidation, ridicule, coercion, threat, mental abuse or other actions of a punitive nature including but not limited to: interference with functions of daily living including eating, sleeping or toileting; or withholding of shelter, clothing, medication or aids to physical functioning.
This deficiency was cleared during today's follow-up visit. The Center Director conducted an all staff training on 02/15/23 to cover the topics of personal rights, behavior management and communication, and mental health and wellness resources available for staff. The Center Director provided the LPA with a sign in sheet of staff members that attended, table of contents, and a print out of the topics covered during this training. Based on corroborating information from staff members interviewed and staff S2's own admission on 02/14/23, S2 violated C1's personal rights by speaking to C1 inappropriately and yelling at the child . This poses a potential health and safety risk for the children in care.

101238(a) · Buildings and Grounds 101238(a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors.
The center director has made the faucets temporarily inaccessible by shutting the water off, taping up the sinks, and posting "do not use" signs. Children will be provided drinking water drinking fountains located at the center. The Center Director stated that the faucet at "Site C" and "Site E" have been replaced and will be retested on 12/22/22.The Center Director stated that she will submit the retesting documents within 2 weeks of the completed sampling to CCLD. Based on record review, the facility had two faucets "Site C" and "Site E" that exceeded that allowable levels of lead in the water (54 ppb and 7.1ppb, respectively).
