Based on interview and record review, C1 was left unattended for approximately 7-8 minutes, therefore the licensee did not comply with the section cited above which posed an immediate health, safety or personal rights risk to persons in care Facility will provide this citation/report to all parents and will have all enrolled parents and future enrollments for the one year, to this date, sign an LIC 9224 and maintain in child's file. 101438.2(b) · Outdoor Activity Space For Infants (b) Outdoor activity space shall be physically separate from space used by children in the child care center and school-age child care center components.

101416.5(d) · (d) There shall be one teacher to every 12 sleeping infants provided the remaining staff necessary to meet the ratios specified in (b) above are immediately available at the center. This requirement has not been met as evidenced by Director will review Staff-Infant Ratio regulations 101416.5 and submit a plan of operation if facility is understaffed and how facility will maintain proper ratios while children sleep. They can take the awake infants outside for play time, they are currently working on a sleeping schedule for all infants in care, as well as seeing if a tall enough partition can be obtain to minimize the sound throughout the infant room

101212(d)(1)(C) · (d) Upon the occurrence, during the operation of the child care center of any of the events specified in (d)(1) below, a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours.
