The licensee will have her adult step-son who resides in the home complete the fingerprint scan process by 11:00am on 12/10/25. Licensee will email LPA Uribe a copy of the completed copy of the Request for LiveScan (LIC 9163) form no later the due date of 5:00pm on 12/15/2025. The licensee's adult step-son who resides in the home turned 18 on 09/12/2025, does not have an eligible criminal record clearance, and was observed to be present in the home today, which results in the potential risk to the health, safety, and personal rights to children in care.
