Licensee stated she will submit a written plan to CCL by 4/27/2026 regarding the completion of the required courses to meet the director's qualifications, along with current staff to complete their Administrations and relations course to meet the Director's qualifications and to submit proof of courses completed by POC due date. Based on observation, interviews and record review, the licensee did not comply with the section cited above by not having proper director qualification nor having other staff who is qualified to be a director in absence of the Licensee, which poses a potential health, safety, or personal rights risk to persons in care.

101237(a) · Alterations to Existing Buildings or New Facilities. Prior to construction or alterations, the licensee shall notify the Department of the proposed change(s).

An updated fire clearance will need to be obtained and the indoor space needs to be remeasured. Based on observation and interview, a half wall was installed in Classroom 3.
