Director submitted a work order on 06/05/2025 for new padding on the pole. During the visit, facility technician secured the padding with duct tape to cover the metal pole. Director will send proof of new padding to the department by POC due date. The velcro that keeps the padding on the basketball pole no longer stays closed and leaves the pole exposed which poses/posed a potential health, safety or personal rights risk to persons in care.
