  Facility Director agrees to submit a written statement of the steps that will be taken by the facility to ensure that all required persons are informed of the communicable illness and submit it to the department by the POC due date. There were reported COVID + outbreaks among facility staff and students beginning August 21. 2025 until September 11, 2025. Facility failed to notify parents, health department in a timely manner and take the necessary steps to stop the spread of the illness.
